Private Tennis Waiver - Spring 2022

If you / your child take private tennis lessons from an instructor at Yarbrough tennis center, you must sign up for this private tennis waiver - spring 2022.  This will complete your waivers AND give us emergency contact information should we need it.  After you complete this online registration, You book your lesson and pay your instructor directly.  On the day of your lesson, please sign in a first and last name with your instructor.  Completing this form does not book you a lesson.
